How do you ensure that Thane Krios stays alive in Mass Effect 2?

Whenever I play the suicide mission in Mass Effect 2, Thane Krios always dies. Even if he's loyal and I've used him for the oculus fight, he still ends up dying during the space battle before you reach the collector base. Here is quick quite to keeping everyone alive (assuming everyone is loyal). Upgrade the Normandy in three three defensive and offensive upgrades, talk Jack, Garrus and Tali to get them.

At the start, you'll need a fire team leader and a infiltrator. Pick Garrus (or Miranda) to lead the fire team and Legion (or Tali) to be the infiltrator. Have Jack or Samara be the Biotic Specialist (Miranda will tell you any biotic can get it done, and that is wrong). Have Mordin lead the crew back to the Normandy, and have either Garrus or Miranda lead the second fire team. This is exactly how I did it, and everyone survived.

Doing this, everyone, including Thane, should live.
John S. (ha!) is correct but he failed to mention that their loyalty must be maxed as well. As long as all upgrades to the Normandy are completed, and your characters are all loyal, then you shouldn't lose him. I'm guessing you lost the loyalty of at least one of your team.
